Support Coordination is an NDIS-funded service that helps you understand and implement your NDIS plan. Our role is to connect you with the right services, build your confidence, and empower you to exercise choice and control over your supports.
With our support coordination service, you can expect:

Psycosocial Recovery Coaching
Living with a psychosocial disability can come with unique challenges. Our Psychosocial Recovery Coaching service is designed for NDIS participants with mental health conditions who need support in navigating daily life, building resilience, and achieving recovery goals.
Our recovery coaches bring both lived and learned experience, offering holistic support that includes:
- Building capacity and independence in managing your mental health
- Creating a recovery-enabling relationship built on trust and respect
- Developing and implementing a recovery plan based on your strengths
- Coordination of mental health supports, services, and community participation
- Motivational and emotional support, to help you stay on track and grow
We work with you, your support network, and clinical providers to ensure your journey toward recovery is supported in the most compassionate and effective way.


Specialist Support Coordination
Specialist Support Coordination is a higher level of NDIS-funded support designed for participants with more complex needs or those facing significant barriers to accessing services.
This service is ideal for individuals:
- Experiencing crisis situations
- Living with multiple or complex disabilities
- Engaged with multiple service systems such as justice, child protection, mental health, or housing
Our Specialist Support Coordinators work closely with you and across different systems and services, to:
- Navigate complex support environments
- Resolve points of crisis and reduce risk
- Coordinate care with health, legal, or community services
- Ensure all supports work together to help you achieve your NDIS goals
At Link Me Support Coordination, we offer both Support Coordination and Specialist Support Coordination, depending on your individual circumstances. We’ll assess your situation, listen to your needs, and provide the right level of support to move forward with confidence.

Psychosocial Recovery Coaching is a specialised support for people with psychosocial disabilities (mental health conditions). It combines the principles of recovery and coordination. Our recovery coaches work closely with you, your support network, and clinical providers to help you manage daily challenges, build resilience, and work toward your personal recovery goals.
Support Coordination is not automatically included in every NDIS plan. It’s usually funded for participants who require help in understanding and using their NDIS plan effectively. Your eligibility depends on your individual goals, needs, and circumstances, and is decided during your planning meeting with the NDIA.
Yes! If you’re not happy with your current Support Coordinator, you have the right to switch at any time. At Link Me Support Coordination, we make transitions easy and respectful—so you feel fully supported without any disruption to your plan.
While both roles help you connect with services, Recovery Coaching specifically supports people with psychosocial disabilities and focuses on mental health recovery. It includes coaching, capacity building, and stronger collaboration with mental health services, whereas Support Coordination is broader and focuses on all types of supports in your NDIS plan.
